I get an error when i try to test a template i created,

I'm a beginner so please dont hold back from pointing out the obvious!

i included a snapshot


please help me out!

Many of us can't or don't want to download files from unknown source.
Post all relavant information in text inside the post.

First, Please read OraFAQ Forum Guide, especially "How to format your post?" section.
Make sure that lines of code do not exceed 80 characters when you format.
Indent the code, use code tags and align the columns in result.
Use the "Preview Message" button to verify.
Also always post your Oracle version, with 4 decimals.
